We are pleased you chose The Spine and Pain Institute of New York for the diagnosis, treatment and management of your pain. To acquaint you with our office policies, we provide the following information.
We request that your referring physician fax results of pertinent studies (i.e. MRI, EMG) and previous care to our office. This aids in optimizing future treatment based upon your complete medical record.

You may fax any medical records or a copy of your insurance care prior to your appointment or bring the completed forms and your insurance card the day of your appointment.
If your insurance requires prior authorization for a visit to a specialist, you must also obtain this authorization prior to your visit.
We accept most insurance programs and their subsidiaries. Please contact our office for a listing of the most current coverage.
If we do not participate in your insurance plan, please speak to Ashley Sullivan or Diana Mosca at (718) 667-3577 to find out if other arrangements are possible.
Insurance co-payments are due at the time of arrival. We accept cash, checks, and credit cards.
Appointment Cancellation and Rescheduling
Unfortunately, we cannot accept walk-in patients, but same-day and next-day appointments are often available.
If you are a new patient, please arrive 15 minutes prior to your scheduled appointment time to allow time to register.
If you are scheduled for an injection or procedure, please bring a designated driver to transport you home. If you are unable to arrange for a driver, please let us know so we may assist in your transportation.
Medications & Refill Requests
Please discuss your medication needs, including refill requests, with our physicians at the time of your appointment. Medications should be taken only as prescribed.
Please do not request early refills of your medications. Patients are responsible for lost or stolen medications and prescriptions once they leave the office.
